All vendors (New or Current) must provide the following documents to be able to participate in any of our festivals;

1. Signed Hold Harmless / Terms ad Conditions

2. Certificate of Authority (Sales Tax ID)

3. Certificate of Insurance (COI) - Food Vendors ONLY

4. Food Protection Certificate - Food Vendors ONLY

5. Temporary Food Service Establishment Permit (TFSE) - Food Vendors ONLY

All vendors must have a Certificate of Authority (Sales Tax ID #).
This is completely FREE.
To apply for a Certificate of Authority:

• Visit the NYS License Center website

• Select the Business link on top of the page

• Log in or create an account

• Select Apply now for a new license or permit

• Search for Application for a Certificate of Authority

All Vendors must provide proof of liability insurance.

• Before registering please provide Clearview a Certificate of Insurance listing Clearview Festival Productions, Inc. 630 9th Avenue Suite 417, New York, NY 10036 as additionally insured.
